MOBILE, ALA. — U.S. Coast Guard officials say pilot error has been identified as the primary cause of last year's deadly helicopter crash during a training flight over Mobile Bay.
Four crewmembers were killed in the Feb. 28, 2012 crash.
The Coast Guard in a statement Friday said there also contributing factors including a lack of established procedures for inadvertently entering instrument flight conditions.
Coast Guard officials say investigators concluded there was no misconduct involving the crash of the MH-65C helicopter.
The four crewmembers killed were Lt. Cmdr. Dale Taylor of Snow Hill, N.C., Lt. j.g. Thomas Cameron of Portland, Ore., Chief Petty Officer Fernando Jorge of Cypress Valley, Calif., and Petty Officer Third Class Andrew Knight of Thomasville, Ala.
