GUARDIAN GROUNDED The mine countermeasures ship Guardian ran aground Jan. 17 off Tubbataha Reef, a World Heritage Site southwest of the Philippines. Navy investigators found an inaccurate chart combined with multiple errors by navigators and watchstanders to cause the grounding; the ship's commanding and executive officers were among those relieved after the incident, which led to the ship's dismantling, angered the Philippines and cost the Navy nearly $90 million.
