Report: Port Royal CO had little sleep

HONOLULU — A Navy Safety Investigation Board report says the commanding officer of a guided missile cruiser had only 15 hours of sleep in three days before the vessel ran aground on a coral reef off Honolulu International Airport.
According to the report obtained by The Honolulu Advertiser, Capt. John Carroll was qualified for the position but was not proficient.
The Feb. 5 accident caused an estimated $25 million to $40 million in damage to the $1 billion Port Royal.
The board’s investigation also found faulty equipment and an inexperienced bridge team contributed to the accident.
Carroll was relieved of his command after the grounding. He was given nonjudicial punishment for “dereliction of duty and improper hazarding of a vessel.”

The cruiser Port Royal ran aground about a half-mile off Honolulu in February after finishing its first day of sea trials following a routine maintenance trip to Pearl Harbor Naval Shipyard.
